Notice of Privacy Practices

This notice describes how protected health information (PHI) about you may be used and disclosed and how you can get access to this information. Please review it carefully. This notice applies to clinical therapy services provided by Melissa Paul, LCSW. It does not apply to coaching services, which are not covered by HIPAA.

Our pledge

We are required by law to maintain the privacy of your PHI, provide you this notice of our legal duties and privacy practices, notify you in the event of a breach of unsecured PHI, and abide by the terms of this notice currently in effect.

How we may use and disclose your PHI without your written authorization

• Treatment. To provide, coordinate, or manage your care.
• Payment. To bill and collect payment for services, including through Headway or your insurance plan.
• Health care operations. For quality, compliance, and administrative activities of this practice.
• Required by law. Including mandatory reporting of suspected abuse or neglect, response to court orders, and reporting required by Colorado or Texas law.
• To prevent a serious threat to health or safety of you or others.

Uses and disclosures that require your written authorization

Psychotherapy notes, marketing communications, sale of PHI, and most other uses not described in this notice require your written authorization. You may revoke an authorization in writing at any time, except to the extent we have already acted on it.

Your rights

You have the right to:

• Request access to and a copy of your PHI.
• Request an amendment to your PHI.
• Request an accounting of certain disclosures.
• Request restrictions on uses and disclosures.
• Request confidential communications by alternative means or at an alternative location.
• Receive a paper copy of this notice.
• File a complaint with this practice or with the U.S. Department of Health and Human Services, Office for Civil Rights, without retaliation.
